Update: i got my card!!! Issued 12/13 arrived today. (12/26) Submitted app online 10/6.. Doc submitted his paperwork 10/10.

10/6 submitted paperwork online with payment 10/10 Dr faxed his paperwork 11/28. received response from my inquiry email 11/27 that my application was received and they process in order of receipt. No date specified 12/19. still waiting.. hopeful based on the last week of posts it...
