ACMPR renewal forms

Hey guys, I'm looking to be pointed in the right direction here...

I have a valid production license til the end of next month and I just got my prescription renewed by my doctor for next year. Obviously,I want to fire it off in the mail to HC asap as it will take forever to be processed.

With my new prescription I have gotten an increase in my g/day and I am hoping to change the address of my grow to another address (where I don't live but have owner's permission) and to change from indoor to outdoor. So, an increase in g/day, an address change for production site and as well go from indoor to outdoor.

Can someone please link me to the forms I need to print and send in for this? I'm paranoid I'll either print off old invalid forms or just the wrong ones.. Having never renewed before, I'm guessing I just fill out what I did initially last year as if I'm brand new and check renew and add in my account # and change grow address and type?

There aren't specific forms for new acmpr patients vs renewal forms are there?

Am I correct in thinking that the whole "Amendment" section is just for people wanting to make changes to their current valid license opposed to wanting to renew their next license with changes? Or does "Renew" only apply if your g/day and production type and site hasn't changed?

I am feeling that I just select renew and skip the amendment section and make the changes in Annex A section.

Use the form you linked, check it as renewal and fill out the amendment section as well as the remainder of the form. Be sure to get signed consent from the owner required under a2

Current wait appears to be 5 months for renewals/amendments from my experience. There’s anectdotal evidence proving that the closer you are to expiration the quicker the renewal is processed.

Thanks Brewery. So basically, if you are renewing but anything at all is changing, you need to select "renewal" AND "Amendment". And for one reason or other spell out what the changes are and why you are making such changes..

I called HC today and was blown away that their phone rang right away and was answered after maybe 4 rings..no hold music at all. Impressive. No 60+ min wait on hold. I was told that I should check the Renewal box even if there are changes compared to last year's application. She said it's perfectly fine to check Amendment box as well and write in the changes, but it isn't necessary since they will be going over everything with a fine-toothed comb anyways.

Nice to see that they have (perhaps) hired more people to work the phones. I'm pretty sure my Leafly app on my phone alerted me today with a notice saying that there are officially 200,000 Canadians enrolled in the medical program(s).

well at least you didn't have to cough up 1/2 a day...to get the bums rush....
