clones dieing after they have roots??

I was hoping that someone could shed some light on my cloning problems. I have only ever had success by taking a very large number of cuttings and getting a low success rate. Right now I have a plant that I re-veged because the smoke was great. The problem is it only has 6 healthy grow tips and with my success rate (2 or 3 out of 25) I risk getting Zero cuttings.
I have read the instructions like 100 times, and many other posts about this here and other places.
In one recent experiment I took 15 cuttings and put them under 2 2foot florescent t5's. I put them in a cardboard box. The cuttings were put in the same coco I use for my plants in an ebb & flow system. Anyhow here is the interesting part: After 2 1/2 weeks they all looked fairly healthy, but one looked a little less healthy. I pulled the least healthy one out of the pot, intending to throw it away, and it had tiny roots coming off the bottom of the stem. Several of the other ones which looked better did not come out of the coco with a little tug. So these must have roots. Yet they do not grow, and if I take the humidity tent off they wilt overnight. It seems like maybe my plants are rooting but not surviving after.

Now a 2nd experiment; I took 7 cuttings in an airogarden. In 2 days several plants showed some little roots. 2 more days and the roots seem no bigger. Its like the roots form and then do not grow. Well I am 4 days in to the 2nd experiment.
