the very top of plant is dying any idea what causes this

Did you burn it with the light maybe?

that looks pretty fucked up. I see a major Potassium def working its way in, and being so young this is probably from salt build up, especially if u r using hydro. and/or u have some root zone issues-that is pretty bad-but what Boneman said, light fall?
