I have a cheaper alternative if you are willing to travel to Farmington Hills. 125 bucks.
He would spend the $75 he saves, on the gas to go back and forth across the state. Not to mention it would take all damn day.

2 hours 35 mins, 148 mi, one way.

let me know how that doctor works for you, im currently looking for someone in the kalamazoo area too but willing drive to save a little money

They want you to have your own Primary but if ya don't have any records they will diagnose for certain conditions, mainly pain. They give you a pain log that you fill out over 3 months and 3 visits(all still 200)

how was dr. dave? i made an appointment for last friday but canceled a few days before because i was worried he would just take my $200

i might reschedule with him though if you had success. any suggestions for when i go in there?

I'm pretty sure that you don't pay if you don't get certified.
You said that you sent in medical records. If your condition is not one specifically listed by name like Hep C, such as pain, then they want 3-4 visits out of ya between them and your primary care physician. If you sent in 3 visits worth of records and they set up an appointment with you.....you are getting your certification. It's almost just a formality to go in there so that he can say he saw you with his in own eye, the information he needs, if you sent in 3 visits, is in those records.
