Well, there are MANY answers to this question...first, nute lock is when the roots cannot absorb the essential nutrients...this can happen for a variety of reasons...its like asking what is being sick? And what causes it? There are many causes and thus, many effects...I recommend surfing this site...I'm SURE there are 1000's of photos with all sorts of nute issues.

check the ph, theres an optimal pH for both soil and hydro, at that pH, the plant can freely absorb whatver you give it

the issue comes when you notice its not taking in an element, say iron for example, so you add more iron, and more iron, and more iron. then you check PH, see that its off, fix it, and you forgot to flush so your plant just got burned with 8 doses of iron.

also, you wanna flush as little as possible, drowning the plant is never a good solution, but its better than burning them, heres is another chart that helps too with identifying whats going on Deficiency.jpgi like charts/diagrams/whatever.
