Renewing Medical Card

I went to a new doctor today to get my two year renewal, and I noticed that on his form that I am submitting to the state, he labeled my condition incorrectly. He checked the correct boxes in each category, however at the end category he manually fills in the approved medical condition, he wrote down the wrong medical diagnosis. I was struggling to say the word and he finished my sentence for me and I must not have caught it since the words sound close. He checked out my neck and spine and then approved me for my renewal and scheduled my next visit in 12 months for a follow-up. Normally, I would go back and correct the situation, however, I kind of delayed getting my renewal which expires in two days, and I am working both of those days. Should I just submit the documents that I have received already, or let my license expire and get it corrected later? In the worst case, would it cause me any issues if I were to ever appear in court for anything related to my medical license?

was it pretty close to your medical diagnosis? like "neck injury causing severe debilitating chronic pain" or was it something completely different?

i'd say just go back to the dr and get a fixed form. if thats possible for you. as it only takes the dr another 2 mins to write a new form.... they usually dont give you trouble about doing it, call the receptionist to verify this...

to answer your question, yes, absolutely it could cause trouble in court. the prosecutors are insane and some of the rulings against patients have been horrendous.
