what is proper documentation?

My brother went to get certified to day. He brought x-rays of his back,collarbone, and leg. and they said that was not enough documentation. He has chronic pain from fracturing vertebrae, a 15 degree collar bone break and a broken leg he walked on for a month causing a calcium deposit to form on the break. He has not had any doctors visits for this condition in about 3 years because there is pretty much nothing they can do for him except give him opiates. My question is What will he have to do to get proper documentation?

That is what I used sorta, I actually just signed medical release forms for chart notes from an surgery I had 18 years prior because of numbness I still feel in the finger where the tendon was sliced in two and had to be reattached.

I also brought in chart notes from a car accident. I had contusions in my neck and vertebrae from the accident an a torn HCL in my knee. This happened a year earlier, but I still regularly have pain and muscle spasms and can't turn my neck or sit on the floor when the weather changes.

What area are you in? There are more doctors in the Detroit area to increase his chances
