I cant really tell you what your doing wrong! ive got about 80-90% success and this is what i do...
Cut off plant, then slice at 45 angle. Then i dip into normal standard tap water with a drop of superthrive.
I then just slice a piece of skin off botten of clone, sort of wounding it. Then dip asap into clonex. I use peat pellets and pre soak in norm superthive water. i also inject a few drops of clonex in the the pellets. Then into the propagator and give em a spray.
all the best

use a humidity dome ontop of a heatmat or something nice and warm, this will encourage ur roots to grow down, plus mist them regularly, sorry if ive mentioned things ur already doing , good luck!!!!!!!!!!!

u can also notch a fiew 45degree angle cuts into the bottom of the clone to encourage faster root development, but i think heat underneath te pots is the key, around 76' to 80', good luck man let me know if u do well. taffo;-)

I wouldn't even use rockwool. It gets kind of overly wet really. Why screw with that shit when you can just use Oasis cubes with perfect results and no pH adjustment needed? With them, put them on a flat surface covered with a layer of paper towel, with a strip of folded up paper towel leading from the main piece into a small water reservoir. This lets just the right amount of water to wick up into the cubes all the time. The whole thing is covered with thin clear plastic. When you see roots growing out the bottoms of the cubes you can pull the cubes up and the paper towel tears away with the cube. You can plant it with the small piece of paper towel still adhering so you don't have to disturb the roots.

I have 100% success rate with that system. The good thing about the cubes is that when you push the cutting in, the end stays in full contact with the foam, so it is supplied with water. Since rockwool is vertical fibers, it doesn't really make good contact with the cut end of the stem, just the sides.

You didn't mention what you're using for light. It has to be pretty low level, lower than you would probably think. Something else is you said the humidity was only about 40%, which is quite low for cuttings. I don't know why it would be that low in a dome.
