How did i grow a mushroom

Not sure how this happened I got home from work and saw this sticking out. Only used water and maybe a tiny bit of gh 3 part.

What soil are you using?

Oh yea sorry forgot that part it is coco with perlite I do keep the rh really high in this seedling dome aswell.
