There's what appears to be alot of paper work, when in reality, it's not all that much, I'll tell you exactly what forms need to be filled out.
Scenario A) You Rent, and the location you rent is your primary residence, and is also your planned production site
1) Form A: Application for Authorization to possess dried marijuana.
2) Form B1: Medical Conditions (or B2: Medical Conditions, if you have Category 2 conditions, accompanied by a diagnosis from a specialist).
3) Form C: Application to produce dried marijuana.
These are extra's, and while it's entirely garbage, I also recommend
4) Form E1: Application to obtain dried marijuana.
5) Form E2: Application to obtain Marijuana Seeds
4 and 5 would be to have access to HC's supply of dried marijuana or marijuana seeds, which I highly doubt you'll actually ever use, but is something that one might consider as a last resort should nothing else be available.
The price that Doctors are charging range from $250-$400, I have yet to hear of anyone charging over $400.
Personally, I paid the $400 to have my situation looked at by a doctor who was not being compensated for their time by HC, like any other doctor would be for any other reasons. Nothing Medical Marijuana is covered by our canadian health plan, this includes, but is not limited to, the Doctor visit that results in the signing of the MMAR form B1 or B2. With the new changes, and to be honest, I'd do it again in a heart beat, the amount of stress I havn't felt since obtaining my license makes it worth 10X the money I paid, however, any more than $400 borders criminal activity. Anyone who charges a simple $400 is covering cost's of staff, equipment, etc, they have a whole brick and mortar foundation to work under, which means they have massive overhead, is simply trying to be compensated for their time, lord knows HC isn't going to compensate them.
Scenario B) Your production site is a rental for the express purpose of production, you do not live there.
1) All forms that fall under Scenario A
2) Form F: Consent from Landlord - This form is only needed to be filled out if you rent, and do
not live where you grow.
