What is the best route to get my card?

Bit of a back story-

At the age of 9-11 i was being tested every couple of months to see if i had a muscular problem, results were i was more than 50% weaker than kids my age. I went from specialist to specialist like everyone ends up doing, having needles stuck into the muscle and being shocked to see how my nerves reacted. Along with a surgery at the age of 13 which came back inconclusive. At this point they were telling me it might be genetic and passed onto my kids and that it also may be degenerative. Ive missed school/work because when i get sick i get sore to the point i cant get out of bed. Now three years ago i started getting the "pins and needles" pain associated with Carpel Tunnel and now need to get surgery which i dont have the time or the money for (missing work is the money part, surgery would be free :)). There is countless hours of sleep that have been lost because of my pains.

Now im 21. Ten months ago i started smoking and loved it for its happy and giggly feeling. The one thing i noticed after the first night was that i was fully rested for the first time. At the time i was only smoking a few times on weekends and i could tell a day or two later when the pain came back, now smoking daily and using the computer less i only get pains while at work and once or twice at home which are very bareable but i can still tell when i stop smoking for a few days on vacation (refuse to drive with weed on me, just not with my luck) my joints(body:-() get stiff and the pain comes back. I notice i become much grumpier after two days because of not getting a good nights sleep. Right now i am working a job where for eight hours straight on my feet, i lift 10-50lb objects and moved around and up and down all the time. If i smoke a few times during the day while not at work i am pain free. This would of been a impossible dream a year ago. Being more active from not having these pains and smoking, i have lost about 50-60pounds in the last 10 months, i have gone from very unhealthy to healthly. I havnt been to a doctor about the muscular problem since i was 15(?) but i have been into the doctor for related injury's/sickness for it. i have been to the doctor recently for the Carpel Tunnel though. I just didnt see the point on redoing all of the tests when the doctors said the same tests would just be repeated.

Currently i am getting overpriced swag in my area (20minute high with a 2hour headache after) and i have gotten weed laced with something once before. Who knows what it was but i puked and was one digit from calling 911, i thought i was on deaths bed. I have two people i can get it from and one is horribly overpriced chemical weed that you cough up a lung smoking but they always have some, With the second having better stuff at better prices but its random when they have it and they also sell/use hard drugs so fearing of getting something laced is always in my head. I dont smoke much only 1-3g/day and if im traveling or doing anything its usually none at all.

In short;
Im 21, i have no criminal past with years of medical visits connected to my muscular problem but less since my use of cannabis. Im not going to go in and ask for a 49plant card, 15 would be more than enough. All i want is to stop buying a bottle of 150 Tylenol every few weeks and kill my liver, grow for my own personal use. To be able to take a vacation and smoke while im on vacation so im not tightly wound the next day. stop fearing of being busted or getting laced herb. I know there is far more people with bigger needs for getting legal MJ than me, but it cant hurt to try and help myself at the same time.

What are my chances?
Could anyone give me the name of a MM doctor in Alberta that would be willing to hear my case?
Is there any way i should start all this if there is a chance?

Thanks for reading and any help you can give. I hope its not to scatter-brained of a post.

Best thing you can do is start with your family DR. Hopefully he is the one who knows your past. Be totally honest with him, and explain it like you did above. He will have to be compassionate to MMJ or you are SoL. The way I started was by contacting Health Canada, and got them to send me an application booklet. Have a read through it, and see what it actually is that you DR and specialist will have to sign off on. It will give you a better idea of the steps you will need to take.

I cant remember the link but i saw it on the forum somewhere, there is a waiver of liability that you can have your doctor sign that basically clears them from any law suits and stuff should something happen with MMJ. I heard sometimes that helps put them at ease a little when Px your MMJ.

PS. I found this website that has the canadian forms on the right hand pane. ( Included the waiver i am talking about )
