Its wrong to put up a down syndrome baby next to an advertisement of planned parenthood, next to a hanger. We all get the joke and its wrong. Not because abortion is wrong, (which it isnt) but the whole thing together is highly offensive.
Michael J Fox on the other hand, well, I feel for him, but I say he's had a better life and continues to do so more than a child with down syndrome.
The offensive nature of something is purely subjective. For me the humour of the image far outweighs any moral or ethical no-no :D

What happens if someone makes fun of a disability they themselves have? What do they get classed as? On from that, do you know 100% that they do not suffer the disability they might be mocking to be able to call them a coward?

I think everyone is fair game because everyone has acted or has the ability to act like an asshole. You dont know how many bitter, just plain mean, people Ive met with disabilities. Even though Ive seen some of the crap disabled people go through, Id say 75% of the time people go out of their way to be nice to them and the one time someone forgets, they invoke the wraith of disable God to complain about it. How many fully abled people can say someone held a door for them or even said hello?

Even with disabilities laugh at themselves sometimes... if they are able to. I think it depends on the degree to which they are disabled, that depends on the degree to which they are 'fair game', but to be honest, ridiculing anyone is mean. It also depends on the aspects of their disability that is being focused on.. Some things are a bitch and they may joke about it to make it easier, but common sense should tell you when that shit isn't funny'. I take the piss out of myself all the time as a reaction to bullying, and sometimes it does help in a way.
