Acmpr medical document question

Hey guys

I am looking to get my Acmpr grow license, I have receive a script for 5gs per day but I am worried they put it on the wrong form.


this is the form I received from NP
can anyone tell me if this is the proper medical document to send with the health Canada application cause I have seen a different style form

You send the original script with the HC paperwork.If your going to grow for yourself,it's 6 pages

Just like tom said, that is the right form but your doctor needs to fill it out(the one you posted looks empty) and then you send it in with your Health Canada application.

Late reply but that is correct. Your Doctor will write out the prescription on that form, and you fill out the rest of the paperwork being the application as per above reply. Take your time filling it out and if you can have another set of eye proof it for you. Cause if you miss signing a spot or date, oit will be returned and you will have to edit and re submit.

I filled out the form myself and sent it in and it was rejected because it must be sent by your doctor. Then they set up a prescription consult with a grow doctor and it was easy. He filled out the paperwork and they sent me the script.

It took 5 months and 3 days from when he submitted it. The cost was zero.

5 months for a new application... crazy. Was this somewhat recent or last year?
