But, I was wondering I was at my Doctors today.. So he did renew my license, but I was wondering about the caregivers license which he was not sure about.. He told me he would sign the forms etc, but I have to get them for him? Are there actual forms, are how would one go about getting one? I have someone that helps me with most of my daily tasks, and would like his assistance on helping me with growing as well.. So how do I keep him out of trouble? If someone could post a link to a legal form my doctor could sign that would be fantastic..
