Application question.

For those of you who saw a Dr. once and had them fill out the application form, what did they write for, "length of time patient has been under your care 9years/months)."? Did they write 0/0 or 0/1 if you saw them once? My doctor wrote 0 since I only saw her once and I'm worried my application will be denied because she wrote 0 months.

Did the doctor review your medical history and give you an examination?

My condition had been documented over some time by my regular physician. But the hospital refused to let her write the recommendation. So I took my records with me to 1body1life in Naperville. All went well and I had my provisional card in no time at all.
You should be just fine.

Yes he did and except me as new p don't know if that made a difference but it seemed to me if you paid the money you are goldin best of luck

Sorry Jeff, wasn't asking you. Thought I hit the reply tab to the op but didn't.
I haven't heard of anyone getting rejected though. On a side note- I do like the new cards better than my old one. Much nicer with a slick finish.
